Compensation: $28.00 – $29.00 per hour

Role Summary

The Clinical Support Specialist provides critical administrative and clinical support to our Care Management team. You will act as a primary coordinator for patient needs, supporting Registered Nurses (RNs) by managing Home Health, Durable Medical Equipment (DME), and pharmacy orders to ensure seamless patient care coordination.

Key Responsibilities

Patient Care Coordination: Manage home health referrals, schedule visits, and coordinate care between patients and external agencies.
Clinical Logistics: Process medication and pharmacy orders with high accuracy; coordinate the end-to-end delivery of Durable Medical Equipment (DME).
Administrative Excellence: Maintain the daily census and visit cadences while handling patient inquiries with professionalism and compassion.
Data Management: Input patient data, vital signs, and treatment outcomes into Electronic Health Records (EHR) following HIPAA and OSHA standards to ensure data integrity.
Team Collaboration: Contribute to a positive and supportive team environment, fostering collaboration to achieve the highest levels of integrity and professionalism.

Requirements

Education: High school diploma or equivalent.
Experience: 3+ years of experience in a clinical setting.
Skills: Proficiency in EHR systems, medical terminology, and strong verbal/written communication skills.
Soft Skills: Exceptional organizational and time management skills with the ability to work independently in a remote setting.
