Location: Remote (Must be based in the U.S.)
Compensation: $28.00 – $29.00 per hour

Role Summary
The Clinical Support Specialist provides critical administrative and clinical support to our Care Management team. You will act as a primary coordinator for patient needs, supporting Registered Nurses (RNs) by managing Home Health, Durable Medical Equipment (DME), and pharmacy orders to ensure seamless patient care coordination.
Key Responsibilities
- Patient Care Coordination: Manage home health referrals, schedule visits, and coordinate care between patients and external agencies.
- Clinical Logistics: Process medication and pharmacy orders with high accuracy; coordinate the end-to-end delivery of Durable Medical Equipment (DME).
- Administrative Excellence: Maintain the daily census and visit cadences while handling patient inquiries with professionalism and compassion.
- Data Management: Input patient data, vital signs, and treatment outcomes into Electronic Health Records (EHR) following HIPAA and OSHA standards to ensure data integrity.
- Team Collaboration: Contribute to a positive and supportive team environment, fostering collaboration to achieve the highest levels of integrity and professionalism.
Requirements
- Education: High school diploma or equivalent.
- Experience: 3+ years of experience in a clinical setting.
- Skills: Proficiency in EHR systems, medical terminology, and strong verbal/written communication skills.
- Soft Skills: Exceptional organizational and time management skills with the ability to work independently in a remote setting.
