Summary Description This position is for a full-time Registered Nurse, Care Manager (CM) Manager, who is responsible for the day-to-day leadership, coordination, and oversight of the Care Management Nursing Team. This role supports the implementation of nursing programs that ensure clinical excellence, patient safety, and a positive care experience across all markets. The RN Manager will work closely with the Senior Director of Nursing to help operationalize quality initiatives and nursing workflows, ensuring alignment with organizational goals. |
Key Responsibilities: - Supervise the daily operations of the Care Management Nursing Team, ensuring consistent delivery of high-quality, patient-centered care.
- Serve as a primary point of contact for nursing staff for clinical guidance, escalation, and operational questions.
- Manage scheduling, caseload assignments, and resource allocation
- Collaborate with the Senior Director of Nursing to implement care standards, protocols, and policies that promote patient safety and satisfaction.
- Monitor and report on nursing team performance, including call cadence, documentation quality, and care coordination outcomes.
- Assist in onboarding, training, and mentoring of new nursing team members.
- Participate in interdisciplinary meetings and collaborate with other departments to ensure smooth care transitions and resolution of patient needs.
- Support the quality and patient safety program by participating in root cause analyses, chart reviews, and process improvement initiatives as assigned.
- Review trends in patient outcomes, readmissions, emergency visits, and care gaps to guide clinical priorities.
- Manage and resolve escalated concerns from patients or their families.
- Help identify gaps in workflow, communication, or training, and proactively recommend improvements to leadership.
- Assist in after-care and clinical triage oversight as needed, ensuring timely and appropriate responses.
- Maintain knowledge of organizational policies, clinical guidelines, and national quality initiatives relevant to nursing practice.
- Be available to rotate on call weekends for supervising daily operations.

Requirements - Active RN license in good standing (multi-state or applicable to primary market)
- Minimum 5 years of clinical nursing experience, with 2+ years in care management, case management, or triage nursing
- At least 2 years of supervisory or team lead experience preferred
- Strong knowledge of healthcare documentation standards and patient safety practices
- Proficient in using EHR systems and communication tools (Google Suite, MS Office, etc.)
- Experience in a startup or fast-paced healthcare environment preferred
