Clinical Liaison (NOLA)
myLaurel is seeking a dedicated and experienced Clinical Liaison based in New Orleans, LA to help launch and expand our innovative home-based acute care services within a leading partner hospital system.
You will leverage your deep understanding of hospital operations, patient flow, and discharge processes to identify appropriate patients and facilitate their smooth transition to myLaurel's specialized home care programs. Your expertise in care coordination, case management principles, and relationship building will be crucial for success. You will act as a key liaison, ensuring continuity of care, preventing readmissions, and advocating for patients moving from the hospital back to their homes.

Key Responsibilities:
- Relationship Building: Cultivate and maintain strong partnerships with hospital leadership, medical staff, and key stakeholders.
- Patient Referrals: Proactively identify and develop referral sources within the acute setting to drive patient volume.
- Patient Engagement: Educate patients/caregivers, ensuring understanding and agreement to the program, providing discharge instructions, and visit expectations.
- Strategic Development: Partner closely with Client Success to execute a comprehensive growth strategy for the new service line, aligning with the hospital's overall objectives.
- Clinical Expertise: Leverage clinical knowledge to provide expert guidance and support to the healthcare team in advocating and recommending patients for the program.
- Performance Measurement: Track and analyze key performance indicators to measure the success of the service line and identify areas for improvement.
- Collaboration: Work closely with the myLaurel team to ensure alignment with company goals and strategies.
- Documentation: Complete required documentation and manage correspondence.

Requirements
- Active licensure
- Acceptable licensure includes one of the following: Registered Nurse (RN), Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN), Licensed Clinical Social Worker (LCSW) or an Emergency Medical Technician - Paramedic (EMT-P)
- Minimum of 2 years of clinical experience in an inpatient, urgent/acute care or emergency room setting
- Strong analytical and problem-solving skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Consistently meet or exceed Key Performance Indicators (KPI’s) through proactive monitoring and process improvement
Typical Schedule:
- Monday to Friday 8:30-5:00 pm
- Occasional evenings or weekends may be required based on patient needs
Compensation & Benefits:
- Salary Range: $90,000-$105,000 annually; includes productivity bonus based on performance metrics.
