myLaurel is seeking a dedicated and experienced Clinical Liaison based in New Orleans, LA to support the launch and expansion of our innovative home-based acute care services within a leading partner hospital system.

This pivotal role will focus on identifying eligible patients, supporting smooth hospital-to-home transitions, and strengthening partnerships with clinical teams. You'll leverage your deep knowledge of hospital operations and care coordination to make a meaningful impact—improving outcomes, reducing readmissions, and elevating patient experiences.

Key Responsibilities

  • Relationship Building: Develop strong, ongoing partnerships with hospital leadership, medical staff, and care teams.

  • Patient Referrals: Identify eligible patients and develop referral pathways to drive volume.

  • Patient Engagement: Educate patients and caregivers on our model, secure informed consent, and support discharge planning.

  • Strategic Development: Collaborate with Client Success to drive service line growth aligned with hospital goals.

  • Clinical Expertise: Apply clinical judgment to support eligibility decisions and program recommendations.

  • Performance Measurement: Track referral metrics and patient outcomes to drive program improvements.

  • Collaboration: Work cross-functionally with internal myLaurel teams to ensure seamless transitions and optimal care.

  • Documentation: Complete timely, accurate documentation and professional correspondence.

Requirements

  • Active clinical license in one of the following disciplines:

    • Registered Nurse (RN)

    • Licensed Practical/Vocational Nurse (LPN/LVN)

    • Licensed Clinical Social Worker (LCSW)

    • Emergency Medical Technician – Paramedic (EMT-P)

  • Minimum 2 years of clinical experience in inpatient, acute care, or emergency settings

  • Demonstrated success in meeting KPIs and improving care transitions

  • Analytical thinker with strong problem-solving abilities

  • Reliable transportation and ability to travel daily between three hospital locations in the New Orleans area (Kenner, Uptown, and West Bank)

  • Valid driver's license and current auto insurance


Schedule

  • Monday–Friday, 8:30 AM – 5:00 PM

  • Occasional evenings or weekends as needed for patient needs


Compensation & Benefits

  • Salary: $90,000–$105,000 annually, plus productivity bonus based on performance
