Registered Nurse, Care Manager (Remote – Full or Part Time)
Reports to: Senior Director of Nursing
Location: Remote (Must be licensed in NY or LA)
Compensation: $43–$45/hour based on experience
Schedule: Flexible Part-Time or Full-Time, including 8- or 12-hour shifts, weekends, and holidays

About the Role
We’re seeking experienced and compassionate Registered Nurse Care Managers to join our virtual care team. In this fully remote role, you’ll support patients during critical transition periods—triaging symptoms, coordinating care, and providing real-time reassurance post-acute events. This is an ideal opportunity for tech-savvy RNs who thrive in dynamic settings and believe in delivering care with empathy and precision.
What You’ll Do
Deliver virtual, patient-centered nursing care with empathy and efficiency
Triage patient concerns using clinical protocols and sound judgment
Escalate care appropriately and collaborate with NPs/PAs as needed
Coordinate follow-ups, labs, and diagnostics across teams
Accurately document all interactions in EMR and team systems
Partner with clinical and operations teams to ensure seamless care transitions
Contribute to a respectful, high-accountability culture
What You Bring
Active RN license in New York or Louisiana (at least one required)
3+ years of hospital-based RN experience (ED or Urgent Care preferred)
BSN required
Willingness to obtain multistate licenses
Comfort with remote care tech (EMRs, chat platforms, telehealth tools)
Ability to work some weekends, holidays, and rotating shifts
